We aim to make each patient experience pleasant and educational. During your first visit we will have you fill out some forms. These can also be found online. There are four forms. There’s a patient registration form, medical history questionnaire, a consent for dilation form, and a marketing form. Check our website where we have each form in PDF format for your convenience. You can print out the completed form and then bring them to your first visit. Also please bring any medical records that we may need and that we may find our helpful for your first visit.
